Advanced practice registered nurse – hematology/oncology

Job description : the advanced practice registered nurse ( aprn ) delivers medical care to a wide variety of patients. The aprn also examines and treats patients independently and collaborates with other health care professionals. Ensures proper illness and injury management, also disease prevention, diagnosis, treatment, and recovery. May prescribe medications and order diagnostic tests. Advises patients about continuing care. The aprn will work within the oncology service line responsible to execute care in the acute and outpatient care settings.

Core job functions

Performs and documents complete physical examinations and comprehensive health histories. Functions independently to perform age-appropriate history and physical for patients. Orders and interprets diagnostic and therapeutic tests relative to patient ‘s age-specific needs. Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities. Implements interventions to support the patient to regain or maintain physiologic stability. Assists with the provision of care in accordance with facility, state, and federal regulations. Monitors the effectiveness of interventions. Facilitates the patient ‘s transition within and between health care settings, e. G. Admitting, transferring, and discharging patients. Collaborates with multidisciplinary team members by making appropriate referrals. Facilitates staff, patient and family decision making by providing educational tools. Serves as a role ledenpop and gangmaker for staff. Ensures and coordinates standardization of advanced practice process between patient care diensten and collaborative teams. Integrates current knowledge of professional issues, trends in healthcare and technological advances into scope of practice. Participates or leads a hospital committee. Co-leads collaborative team in developing a new service or strategic vertoning improvement. Adheres to university and unit-level policies and procedures and safeguards university assets. Participates in survivorship and long term care of patients following completion of treatment. Department specific functions

Ctu ( chemotherapy treatment unit )

Provide chemotherapy treatment unit ( ctu ) coverage at the main downtown campus. This includes on-call and cross coverage. Provides new patient and ongoing teaching to oncology patients. Able to collaborate with physicians and other providers regarding the care of a patient receiving treatment in the ctu and in all settings including inpatient. Assures appropriate medical documentation. Collaborate with pharmacy and nursing staff to optimize quality of care and time of the patients treated at ctu. Refer to support instituten when needed. Precept or leraar nursing staff and nurse practitioner students/fellows. Provides patient/family education regarding disease processes, treatment options, treatment outcomes, possible complications, medications, discharge programma and lifestyle modifications. Promotes evidence based nursing practice. Participate in unit qi/research projects. Outpatient oncology clinics and/or hospital-based centers

Respond to emergencies, rapid responses, and code blues. This includes weekend call. Participate in the code blue and emergency response committee meetings when requested. Participate in ethics committee and schwartz rounds when requested. Collaborates with nursing leadership, primary clinical team, and support diensten on the care of patients with reported deteriorating conditions. Collaborate with physicians, staff, and leaders in the care of ambulatory oncology patients. Provides patient/family education regarding disease processes, treatment options, treatment outcomes, possible complications, medications, discharge programma and lifestyle modification. Assure appropriate transfer of patient/communication/documentation to the receiving physician and team. Able to discuss advance directives with patient and/or family/caregiver. Assists in the management of family dynamics, coping mechanisms during acute and chronic phases of patient care. Assist with deliver of survivorship care plans and program clinical needs. Participate in weekly survivorship operational committee meetings and other pertinent meetings related. Collects data for patient and program evaluation and participates in quality assessment and improvement activities. Provides coverage to areas of need within main campus. Clinical management:

Responsible for the beheer of the daily operation of patient care, compliance with nursing standards, patient satisfaction, and clinical voorstelling improvement. Responsible for patient flow throughout designated clinical areas. Identifies patient flow barriers both internal and external to the clinical operation, and provides and/or implements improvement solutions. Responsible for assuring patient care is delivered in alignment with hospital mission, and nursing department goals and objectives. Responsible for insuring unit ( s ) are adequately staffed and personnel are appropriately trained for their given assignments in order to provide safe, quality care. Responsible for assuring staff compliance with mandatory education, licensure requirements, continuing education, and applicable certifications and pro practice standards. Responsible for the utilization and maintenance of appropriate nursing care plans and assessment standards. Responsible for assuring unit ( s ) are prepared for snaak commission surveys and inspections from regulatory agencies. Responsible for the collection of data to ensure voorstelling improvement and optimal customer service standards. Provides timely reports as requested. Coordinates with superiors and human resource department the beheer of personnel to include recruitment, retention, employee incentive and evaluation, and disciplinary actions. Provides insight on unit operations to nursing leadership to assist in the development of strategic planning and departmental expansion. Assists with the development/revision and review of clinical policies and procedures. Financial management:

Responsible for capital and operational begroting programma and budgetary compliance. Responsible for timely and accurate data collection of patient volumes, unit expenses, unit revenue, show improvement information, patient satisfaction, and other pertinent information that demonstrates the operations of the unit ( s ). Reconciles expense receipts with begroting report to ensure accuracy. Provides timely reports, as requested, on all data collected. Maintains appropriate records of employee attendance, tardiness, and non-productive time such as sick and vacation time, administrative leave, and leave of absence. Maintains records and systematic management of inventory such as biomedical equipment, capital equipment, supplies, and miscellaneous rekentuig hardware.

Patient and patient family education:

Assures nursing staff assess patient and family educational needs. Assures staff receives adequate education regarding a variety of disease processes, therapies, and new protocols. Assures nursing staff is knowledgeable in arranging consults with other departments as appropriate to address patient changing needs and/or medical status. Assists the staff to utilize and/or adapt teaching protocols to set goals for the individual patient/family teaching plan. Staff competency, education, and voorstelling evaluation:

Assures all staff receive a formal orientation plan and preceptor. Evaluates effectiveness of the general and clinical orientation. Assures completion of competency based assessment documentation. Systematically and continuously evaluates staff learning needs utilizing competency-based criteria. Assures all staff meet mandatory continuing education requirements, and yearly hospital based competencies. Formally evaluates each staff member yearly. Maintains documentation of job voorstelling, personnel actions, and additional professional contributions to the unit and/or organization. Assures cross-training to other departments is documented and accurate. Maintains educational documentation requirements of the hospital. Provides ongoing coaching to all staff with the goal of improving critical thinking and technical skills. Voorstelling improvement:

Assists in the development and implementation of voorstelling improvement programs in compliance with jcaho standards and other regulatory agencies. Collects data to evaluate patient flow and trends. Through data collections, identifies changing needs for internal processes, and implements change accordingly. Collaborates with leadership for processes flow issues external to the nursing unit ( s ). Identifies opportunities for improved unit efficiency, patient flow, and customer service. Investigates clinical and practice variances and conducts follow-up action plans as appropriate. Committee participation:

Functions as an active, competent member of the facility ‘s code blue team, show improvement team and nursing shared governance counsels. Demonstrates knowledge of the quality assurance/performance improvement program for the designated unit. Coordinates unit pi program and submits reports in a timely manner. Participates in shared governance and appropriate practice counsels. Communication/patient satisfaction:

Leads by acroniem to provide service excellence by communicating in a cooperative, positive and professional manner. Displays courtesy toward customers by introducing self and addressing customer by name. Meets customers ‘ expectation by listening, seeking solutions to potential and actual problems, and demonstrating sensitivity and awareness of others. Education:

Master ‘s degree or higher

Certification and licensing:

Dc aprn license graduate of master in science of nursing or higher curriculum. Board certification as aprn from nationally accredited organization such as american nurses credentialing center ( ancc ) or american academy of nurse practitioners ( aanp ) acute care nurse practitioner certification preferred current certification in bls and acls citi certification within three months of employment ons chemotherapy and biotherapy certification on hire or during orientation. Experience:

Ondergrens 3 years of relevant advanced practice nursing experience preferred
